For example, the client may reveal that she is not taking the medication that has been prescribed for her, or even that she is misleading her practitioner about this. Through such a disclosure the counsellor can be placed in an ethical dilemma where there is a conflict between the commitment to confidentiality about client material and a responsibility towards the organisation in terms of a wider duty of care to patients.

Extra effort is often needed to ensure updating on professional and clinical matters and adequate channels for consultation and advice on medical and psychiatric issues. The latter is also important for voluntary sector and community organisations providing counselling. In addition, depending on the background and knowledge base of trustees, there may not be an appreciation of the clinical skills needed to effectively manage the service and not just as part of the clinical work with clients.
